If you've been to the hospital, had an upper endoscopy, and were told "no gastritis" or "no problems," then a more comprehensive approach may be necessary.

Today, we'll explore the hidden causes behind the common symptom of post-meal heartburn, one by one.

1. Heartburn: Why Does it Happen, Exactly?

Patients often describe it this way:

“My stomach feels like it's burning.”

“My chest feels hot/sizzling.”

“Sour liquid comes up and my throat burns.”

These descriptions typically refer to the sensation felt when stomach acid refluxes into the esophagus, or when stomach acid excessively irritates the gastric lining. However, it's crucial to understand that the cause isn't simply an excess of stomach acid.

2. Heartburn Varies Depending on 'When' it Occurs

First, you need to check when heartburn appears after eating.

  • Immediately after eating: This could be an early pattern of reflux esophagitis, where the stomach contents themselves stimulate the sphincter, allowing stomach acid to flow into the esophagus.
  • 30 minutes to 1 hour after eating: This is when stomach acid secretion peaks and intra-gastric pressure rises, worsening reflux. These are typical symptoms of GERD (Gastroesophageal Reflux Disease).
  • More than 2 hours after eating: This could indicate gastroparesis or a functional disorder, where gastric emptying is delayed, food remains in the stomach for a long time, and stomach contents continuously reflux into the esophagus.

3. The Causes Are Diverse; Let's Highlight a Few

  • Fatty and greasy foods → Increase stomach acid secretion and loosen the sphincter.
  • Caffeine, chocolate, alcohol → Suppress sphincter function.
  • Overeating → Significantly increases stomach pressure.
  • Lying down immediately after eating → Gravity disappears, allowing acid to easily rise.
  • Stress, insomnia, tension → Make the gastrointestinal nervous system sensitive.

4. The Differentiating Factor is the 'Combination with Other Symptoms'

  • Heartburn + frequent sour burps: Highly likely to be typical GERD.
  • Heartburn + worsens on an empty stomach: More likely to be gastritis caused by excessive stomach acid or weakened mucosal defense.
  • Heartburn + frequent throat discomfort or hoarseness: Atypical reflux (LPR), which affects the upper esophagus, may also be suspected.
  • Heartburn + normal endoscopy but persistent symptoms: This could be functional dyspepsia (epigastric pain syndrome type) or visceral hypersensitivity.

5. Treatment Prioritizes Understanding Patterns Over Medication

While various medications like PPIs, antacids, and gastroprokinetics can be used, what's more important is this:

  • When do your symptoms recur?
  • Which foods worsen your symptoms?
  • Do you lie down immediately after eating?

Recording these details and adjusting your lifestyle patterns first can be highly effective. In cases of functional issues, the approach should not merely suppress acid, but rather reduce sensitivity and stabilize the gastrointestinal autonomic nervous system.

Heartburn is not simply a problem of stomach acid alone. It is a symptom that results from the combined effect of stomach acid secretion, fundic function, esophageal sphincter status, gastric emptying speed, and sensory sensitivity.

If it recurs, don't just rely on endoscopy results and dismiss it; try to accurately record when, with what sensation, and after which foods it appears after meals. That will be the key to your treatment.
