Restless Legs Syndrome: Can Iron Supplements Help?

1. Legs Feel Like They're on Fire at Night — What is Restless Legs Syndrome?

Some people say, "I was fine all day, but as soon as I lie down in bed, it feels like something is crawling inside my legs."

Or, "I can't stand not moving my legs, so I have to keep rubbing them."

Restless Legs Syndrome (RLS) is, quite literally, a syndrome that makes it difficult to keep your legs still. While relatively less severe during the day, its characteristic worsening at night often leads to sleep disorders and significantly impacts quality of life. It is often considered a dopamine-related issue, sometimes leading to the use of Parkinson's medications, or iron supplements are recommended due to presumed iron deficiency. But is it truly an iron deficiency?

2. Why Does the Dopamine Circuit Need Iron?

Dopamine is a major neurotransmitter responsible for various brain functions—including movement, reward, and emotional regulation. However, something essential is required to produce dopamine: iron, specifically Fe²⁺. The first step in dopamine synthesis is the conversion of an amino acid called tyrosine into 'L-DOPA', a process catalyzed by the enzyme 'tyrosine hydroxylase'. This enzyme does not function without iron. In other words, dopamine can only be produced if iron is present. This reaction occurs in the 'substantia nigra'—a region in the brain particularly rich in dopamine neurons. This area is one of the most iron-concentrated regions and is the core of the dopamine circuit. A reduction in this iron can lead to hyperexcitability or instability in the circuit, potentially causing restless legs symptoms.

3. But... Can Iron Supplements Restore the Circuit?

Many doctors prescribe iron supplements when ferritin levels are low. However, ferritin is an indicator of 'stored iron', and it doesn't directly reflect the state of 'iron used in the brain'. The brain, in particular, strictly controls the influx of external substances through the Blood-Brain Barrier. Iron is no exception. For iron to enter the brain, various transporters such as transferrin, DMT1, and ferritin must function precisely, and if even one part of this process breaks down, iron cannot enter the brain. Therefore, even if blood iron levels are sufficient, the brain itself can be iron deficient, which is the pathological background of restless legs syndrome.

4. Sophisticated Iron Metabolism System vs. Simple Iron Supplement Prescription

Iron metabolism is a system in which the entire process—from absorption in the stomach to storage, transport, and utilization in the liver and brain—is precisely regulated without a single error. Numerous regulators such as DMT1, hepcidin, ferritin, and ferroportin are involved. But what about prescriptions in reality? Often, doctors only look at a single ferritin level and say, "You're iron deficient, take iron supplements," and that's it. This is an overly blunt intervention compared to the structural sophistication of iron metabolism, and in many cases, it is ineffective or only causes side effects. Furthermore, in excess, iron can be a source of oxidative stress. Fe²⁺ can react with hydrogen peroxide to create highly potent reactive oxygen species (·OH), which damage cells. In other words, iron is an essential metal, but also one that must be carefully regulated.

5. Why Do Iron Supplements *Seem* Effective, Then?

Occasionally, some people report an improvement in RLS symptoms after taking iron supplements. Why is that? Indeed, when ferritin levels are very low, below 50 ng/mL, iron supplementation can lead to a certain degree of stabilization in the dopamine circuit. The problem, however, is that most patients do not meet this criterion, or they experience symptoms despite normal ferritin levels. For these individuals, iron supplements offer little benefit or only cause side effects such as abdominal pain, constipation, or poor absorption. In other words, iron supplements are only meaningful for a select few and can be a 'misdirected treatment' for most RLS patients.

6. How Can Traditional Korean Medicine Bridge This Gap?

Traditional Korean medicine does not directly deal with iron as a substance, but it approaches treatment by adjusting the environment in which iron functions—that is, the 'internal conditions'. For iron to be absorbed, stored, delivered to the brain, and function, the spleen-stomach must aid iron absorption, the liver must store and redistribute iron, the heart must regulate sensory circuits and sleep rhythms, and the kidney must create an ascending pathway for iron to reach the brain. Traditional Korean medicine can diagnose imbalances in these four axes (liver, heart, spleen-stomach, kidney) and address them with specific herbal formulas: Hyangsaryukgunja-tang for spleen qi deficiency, Gamisoyo-san for liver qi stagnation, Gwibi-tang for heart blood deficiency, and Jaumganghwa-tang for kidney essence deficiency. In other words, the approach is not to simply add iron, but to create a 'state where iron can be utilized'.

7. Instead of Adding Iron, Restore the Operational Environment

Restless Legs Syndrome may not be a disease caused by a lack of iron. More precisely, it's a condition that arises when iron is present but not properly utilized. Therefore, indiscriminately taking iron supplements is like pouring fuel without understanding the engine's function, and the side effects could outweigh the benefits. The treatment approach now needs to shift from simply adding iron to 'tuning' the body so that iron can function effectively. Traditional Korean medicine can contribute to restoring this delicate balance. A body that can properly utilize iron again, a circuit that regains its rhythm. That is the core of RLS recovery.

Rather than fitting a patient into one perspective, I consider what each can and cannot explain and look for clues that connect symptoms and treatment histories that were previously considered separately.
