Why does atopy appear on the calf instead of the folded areas?
A man in his mid-20s. His atopic dermatitis, which began when he scratched the inside of his elbow in elementary school, returned after a long ceasefire once he became an adult. However, in recent months, the pattern of the battle has changed. The battlefield has shifted. Instead of the inner elbows that had become relatively calm, coin-shaped red lesions about 2-4cm in diameter have been growing one by one on the hard front of the lower legs and on the sides.
“It gets itchier at night. When I scratch it, a thin fluid comes out.”
The phenomenon where the skin rises in round bumps only on the calves and flanks, while the folded areas remain intact. This is the first mystery we must solve, as it is difficult to explain using only the typical map of atopy.

To start with the conclusion, this phenomenon is a joint product of local 'gaps' and systemic 'tone'.
Imagine one giant embankment. If the waterproofing function of the entire embankment has weakened (systemic atopic tendency), water does not seep out from the entire embankment. Instead, water pools like coins only in the 'gaps' that are under the most pressure or are structurally vulnerable. The skin is the same.
His skin was deficient in ceramides and natural moisturizing factors (NMF), resulting in compromised basic waterproofing function. On top of that, there was a 'topographic weakness' in areas like the front of the lower legs that are prone to friction from clothing and dryness. He had a habit of showering with hot water for 15-20 minutes daily, and moisturizing 2 or fewer times per week. Of course, other possibilities must also be confirmed. I always interrogate the most common suspect first. First, fungal infection suspected during prolonged steroid use (concealed tinea). However, the KOH test performed by scraping the lesion edges was negative on both occasions. The fungal model stepped back.
Second, contact dermatitis. If symptoms began after exposure to new detergents, perfumes, or metals, it would be a compelling hypothesis. However, his lesions appeared fixed at specific sites, and it was difficult to find a clear temporal link between contact and symptom onset. Both suspects had alibis.

I unfolded a different map called Korean Medicine. On this map, his condition is divided into two layers.
Gi (基), namely the foundation
His body is close to a state of 'blood deficiency and wind dryness (血虛風燥)'. Because the blood (血) that nourishes the skin is insufficient, it is generally dry, and due to this, the 'wind (風)'—which is itching—becomes more severe at night. The dry appearance of the tongue with almost no tongue coating supports this hypothesis.
Pyo (標), namely the phenomenon
Localized lesions where exudate occurs in a coin-like shape are a clumping of 'Wind-Damp-Heat (風濕熱)'. It is a pattern where moisture and heat tangle locally on dry land, causing an inflammatory response. Modern medicine's 'decline in barrier function' overlaps exactly with the dry 'base (基)' of Korean medicine, and 'localized inflammation' overlaps with 'Damp-Heat (標)'. The two maps were pointing to the same land.
Korean Medicine Approach: Simultaneously managing gaps, itching, and immunity
The integrated diagnosis is a 'complex type where nummular eczema is superimposed on a base of dry atopic dermatitis'. Therefore, treatment proceeds as a process of filling the gaps in the embankment, turning off unnecessary alarms (itching), and restoring the balance (immunity) of the entire embankment.
First,
'Herbal medicine' regulates systemic inflammation and immune responses. This is the process of correcting the body's 'tone,' which is the root of the problem. To improve the dry foundation (hyeolheo-bongjo) and stabilize the immune system, herbal medicines from the Danggui-eumja (Dangguiyinzi/當歸飮子) family are used, and to suppress localized inflammatory responses (pungseup-yeol), herbal medicines from the Sopung-san (Xiaofengsan/消風散) family are used, seeking the body's overall balance.
Second,
Break the cycle of unbearable 'itching' with 'acupuncture treatment.' Itching is not a problem of the skin, but a signal from hypersensitive nerves. By placing acupuncture needles around the skin lesions and at specific acupoints throughout the body, the nerve pathways that transmit itching signals to the brain are directly regulated and stabilized, preventing 2ary damage and the vicious cycle caused by the 'act of scratching'.
Third
Fill the gaps of the collapsed 'skin barrier' with 'Oriental medicine external agents.' Consistently apply Oriental medicine external agents (Oriental medicine ointments) made from herbal medicines that aid in skin regeneration and moisturizing, such as Seupjin-go, Jaun-go, or Cheongdae-go, to the coin-shaped eczema areas. Beyond simple moisturizing, this serves to physically reinforce the 'gaps' in the embankment by directly supplying nutrients to the damaged skin barrier and forming a protective layer.
The gapped barrier and unfavorable terrain create the 'shape,' and the imbalance of the entire body determines 'how many times that shape will repeat.' Ultimately, this arduous struggle is not about dealing only with the visible inflammation of the skin, but a process of redesigning the invisible terrain and overall balance of my body. Now that the map is understood, it is time to set out on the journey.
